Therapeutic drug monitoring (TDM) is generally defined as the measurement and medical interpretation of chemical parameters in blood with direct influence to drug prescribing procedures. Its main focus is on drugs with a narrow therapeutic range, which can easily be under- or overdosed.

There are numerous variables that influence the interpretation of drug concentration data: time, route and dose of drug given, time of blood sampling, handling and storage conditions, precision and accuracy of the analytical method, co-medications and, last but not least, clinical status of the patient.

Many different professionals starting with the physicians over the clinical pharmacists to the nurse are involved in this multidisciplinary process. Failure to properly carry out any one of the steps needed, can severely affect the usefulness of using drug concentrations to optimize therapy.

Therefore Hamilton started to develop together with its Partner Prolab Instruments a workstation which fits easily in clinical laboratory and minimize the human interaction to analyze blood, serum or plasma samples.

Based on the Flip Tube Technology which allows opening and closing of tubes as well as single tube transport, the workstation combines the automatic preparation of the calibration curve with up to 12 free selectable calibration points, the QC curve and the samples. Single sample processing avoids elongated queue time which prohibits further degradation of the analyte during the sample preparation procedures. Furthermore it allows the clinical laboratory to run more different parameter test without the necessity to collect samples.

The workstation is not only preparing the samples but is also capable to control the LC MS system in a way that during the LC MS run the next sample will be prepared for the next injection and after injection the workstation automatically restarts the LC MS for the next analysis. All injection and washing steps of the injection valve are done with disposable tips which minimize the time for the cleaning procedure with carry over rates less than 0.3%. The inter day variation of the analysis is less than 11%.
