In this blog Prof. Fanelli, a world-wide expert on carotid artery stenting and teaching, focus on the website www.carotidworld.org. The site is entirely devoted to carotid pathology.

Stroke is the third leading cause of death and the most common cause of long-term disability in the western world. Roughly 25 % of strokes are due to carotid artery disease. Today, carotid endarterectomy is decided to be the standard of care for management of significant carotid artery disease within the vascular community. However, the accumulation of experience and improvement of endovascular tools such as stents as well as filters, and the strong demand from society for use of less invasive alternative to the open surgery technique has made carotid artery stenting (CAS) an alternative. CAS may be seen as an equivalent choice to carotid endarterectomy when demographics such as patient age and anatomy, surgical risk, and operator experience are considered.

In open surgery, the training method still follows the traditional apprenticeship model in which a supervising consultant bases assessment of the trainee’s performance on subjective observation. The feedback provided to the trainee on their performance also tends to be unstructured lacking transparency. During many endovascular procedures the patients are mostly awake thus the communication between the supervisor and the trainee is restricted which limits per-procedural feedback to the trainee. Often, the order and the complexity of clinical procedures presented to the trainee are random resulting in sub-optimal and inefficient learning opportunities.

An increased focus on patient safety by professional medicine and patients virtual reality simulation training offers enormous potential for training and assessment. The website www.carotidworld.com together with endovascular simulation allows for training and rehearsal outside the operating room and catheterization laboratory in an environment where the patient is exposed to no risk.
