Swedish platform enables Ukraine’s first Remote Patient Monitoring project - the first of its kind-project aims to support the healthcare system during wartime challenges

Shortly before summer, Ukraine’s first RPM patients were connected to the monitoring via the Swedish software platform ImagineCare. The initiative aims to reduce primary care and hospital visits, which currently presents new challenges. The project, which includes patients with heart failure and hypertension, will later be evaluated from three perspectives – health outcomes, economic impact, and reduced environmental impact. An initial evaluation has now been completed and shows, among other things, success in the timely intervention of complications.

As the country’s first remote monitoring project, patients with heart failure and hypertension have now been connected to ImagineCare’s platform for home-based follow-up. The goal is to reduce both disease progression and hospitalizations, as well as the financial and environmental impact caused by hospitalization and healthcare-related travel, such as ambulance transport. The project is run by Primary Health Care Center №4 of Kryvyi Rih City Council, a healthcare provider with more than 70,000 registered patients across 14 practices.

During the first few months of the monitoring, the clinic has seen a clear improvement in disease control and early intervention. Among other things, the following has been observed:

  • Fluid retention detected in 10–15% of patients
  • Sudden increases in blood pressure detected in approximately 12% of patients
  • Early medical intervention successfully prevented complications in all cases

Approximately nine million people in Ukraine suffer from chronic illnesses, including multiple chronic conditions that require continuous care. The healthcare system, which often must prioritize resources for war injuries and emergency care, has an urgent need for innovative solutions to manage chronic diseases.

The project is partly funded by NOPEF, the Nordic Project Fund, and lays the groundwork for continued collaborations in the country.

About PHHC 4

Considering the geographical features of Kryvyi Rih, located in central Ukraine, and to ensure equal accessibility for each district, the city established seven Primary Health Care Centers (PHCCs). PHCC No. 4 is operating since 2011 and comprises 14 general practices to serve more than 70,000 residents, which is the largest number among all PHCCs in the city.

About Remote Monitoring

Remote monitoring allows patients to measure their health values from home while maintaining direct contact with healthcare professionals via an app. In case of any deviations, healthcare staff are alerted immediately, enabling prompt action to prevent serious deterioration – whether related to chronic conditions or hospital care at home (HaH).
